Dementia Together Collaborative

The Cornwall Memory Cafe Network is also a partner in the Dementia Together programme, alongside Cornwall NHS Foundation Trust, The Alzheimer’s Society and Memory Matters.

This service supports people with a diagnosis and their carer, through personalised and holistic support from a Dementia Advisor, and regular reviews to check in that all is well, as well as referrals to your Primary Care Dementia Practitioner, at times of increased need.
